OUTCOME OF SURGICAL TREATMENT OF POSTERIOR WALL AND POSTERIOR COLUMN FRACTURE OF ACETABULUM

  Abstract :

BACKGROUND: Fractures of acetabulum are relatively uncommon but as they involve major weight bearing hip joint in the lower extremity, they have great clinical importance. As the acetabular fractures are intra-articular fractures we aim to reduce the acetabular fracture to regain near normal anatomy with stable fixation. There have been only a few studies which have determined the functional outcome and considered complex and simple fractures of the posterior wall and posterior column separately.1 Therefore we conducted this study to review the results on functional and radiological background in our institution. MATERIAL AND METHODS: A prospective hospital based observational study was carried out on 35 patients to evaluate surgical outcome after reconstruction of posterior wall and posterior column fracture of acetabulum and resulting functional and radiological outcome at short term follow up using by Modified Merle dAubigne and Postel scale. RESULTS: In our study 30 males with the mean age 37.7 years and 5 females with the mean age 52.6 years showed excellent results in7 patients (20%) , good results 23 patients (65.7%) in , fair results in 2 patients (5.7%) and poor results 3 patients (8.5%). CONCLUSION:We conclude that open reduction and internal fixation of the displaced acetabular fractures is effective method of treatment despite complexities and difficulties in approach and reduction. The accuracy of reduction is an important predictor of functional outcome. The approximation to normal anatomy will depends on complexity of the fracture and expertise of the surgeon. Finally even if long term outcome of restoration of joint function is not always good, restoring normal anatomy will enable the patient to have better quality of life in near future and to have successful reconstructive procedures in distant futures.
