Comparison Of Fixed-bearing And Mobile Bearing Total Knee Arthoplasty-a Prospective Comparative Study

  Abstract :

Total knee arthroplasty has grown on a significant high rise during the past 40 years, due to the change in lifestyle and increase in medical ailments. This has led to advanced degenerative joint disease of the knee which has deprived the function of the normal knee.Various factors have changed the modality of treatment in degenerative Osteoarthritis of the knee joint. During this course of treatment conservative management could not play a role beyond preservation of the knee joint. Considering this fact, a new evolution of total knee arthroplasty has been evolved. There by relieving the pain, improvement in the function of the knee joint as well as leading a good quality of life. Multiple researches have been made in total knee arthroplasty; where in two such devices have been in consistent to the normal anatomical and kinematic function of the knee joint. Those devices are divided into two groups. Namely; a) Fixed bearing knee arthroplasty b) Mobile bearing knee arthroplasty. Our study helps to analyze and compare the outcome of both the prosthesis functionally and radiologically. Conclusion-Range of movements in all the patients included in the study showed a significant improvement post operatively.Functional outcome in patients treated with mobile bearing system had better than the patients treated with fixed bearing system.Radiological outcome had a prognostic change after total knee replacement irrespective of the bearing system used for the patients.Fixed bearing patients showed a small but comparatively better prognosis than patients treated with mobile bearing prosthesis after assessing with oxford knee society score postoperatively.Long term study with preferably larger sample size may provide a significant functional and radiological outcome.This study had attainted a probability (p) value of >0.05 which showed an insignificant change in the mobile bearing prosthetic design and fixed bearing prosthetic design to be chosen for total knee arthroplasty
